ADHD medications and cardiovascular health

Taking stock with science… and good clinical judgment

When it comes to medication for Attention Deficit Hyperactivity Disorder (ADHD), concerns can sometimes arise about possible effects on the heart. These are legitimate questions. Above all, they deserve clear answers, based on the best available scientific data.

ADHD medication: increased cardiovascular risk?

What the latest science shows

A large meta-analysis published in November 2022 in JAMA Network Open analyzed data from 19 studies involving nearly 3.9 million patients, including children, adolescents and adults.

The aim was to determine whether medications used for ADHD – whether stimulant or non-stimulant – increase the risk of cardiovascular events such as :

  • high blood pressure
  • arrhythmias
  • heart attacks
  • cerebrovascular accidents (CVA)

The results of this analysis are clear and reassuring: no statistically significant increase in cardiovascular risk was observed:

  • in children and adolescents
  • in young, middle-aged and older adults
  • with stimulants and non-stimulants

In other words, in the vast majority of patients, ADHD medications are not associated with increased cardiovascular risk.

However, the authors cautiously and rigorously point out that clinical monitoring remains important, particularly for certain patient profiles at greater risk. An approach that we at Priveo Santé fully share.

What about children and teenagers?

The Canadian recommendations of CADDRA


CADDRA, the Canadian reference organization for ADHD, reminds us that :

  • Sudden death in young people is extremely rare; approximately 1.2 to 1.3 cases per 100,000 people.
  • The potential link between ADHD medication and this phenomenon remains weak and unproven
  • The benefits of ADHD treatment often far outweigh the risks, when properly prescribed and monitored.

The importance of assessing before prescribing

At Priveo Santé, every ADHD evaluation includes a detailed detailed clinical questionnaireincluding :

  • family history of sudden death
  • unexplained loss of consciousness
  • symptoms related to exercise or exercise intolerance

It’s important to know that routine ECGs are not recommended in patients without cardiac symptoms. Current data show no benefit to routine screening.

Follow-up: a key stage

When starting treatment, particular attention is paid to the appearance of :

  • syncope
  • dizziness
  • shortness of breath or exercise intolerance

In the presence of such symptoms, prompt medical assessment is indicated, and temporary interruption of medication may be considered.

In short, in the typical child or adolescent with ADHD and no known cardiac problems, the risk of cardiovascular side effects remains very low.

ADHD medication and other stimulants: a message of caution

Drugs such as Concerta, Ritalin, Biphentin, Vyvanse, Adderall XR, Foquest and Quillivant do not formally prohibit coffee or caffeine consumption in their monographs.

That said:

  • Health Canada reminds us that mixing stimulants can increase the risk of adverse effects (palpitations, hypertension, agitation).
  • The FDA( U.S.Food and Drug Administration ) points out that the combination of prescribed stimulants with other stimulant substances significantly increases the risk of toxicity.
